Health Insurance (Medical and Vision)

Regular employees who work twenty (20) hours or more per week are eligible for insurance coverage for themselves and their eligible dependents. Insurance premiums for part time positions (20-29 hours) are pro-rated. Medical starts the first of the month following your hire date, unless you started on the 1st day of the month, then coverage will begin the day hire. The city offers the following options for insurance coverage:

Moda Preferred 500

This plan is considered a Preferred Provider Organization (PPO) plan and allows you to visit whatever in-network provider you wish without first requiring a referral from a primary care physician (PCP). Employees are encouraged to select a primary care physician (PCP) from an established list of preferred providers. The deductible is $500 ($1,500 for family) and the maximum out-of-pocket (Moda will pay all costs after you reach this amount) is $1,500 for individual and $3,000 for family for in-network; $3,000 for individual and $6,000 for family for out-of-network. Many providers are included in the network (Providence, Legacy and Portland Clinic). Employees pay a $20 copay for office visits, specialist visits, Urgent Care and acupuncture care & Spinal Manipulations. For most services (example - surgery, outpatient, X-Ray, and Labs) employees pay 15% (Moda will pay 85%). If employees go through an out-of-network provider, the coverage is at 40% (Moda will pay 60%).

Prescriptions purchased from retailers for a 30-day supply are $2 for value, $15 for generic and $30 for brand drugs. Mail order prescriptions for a 90-day supply are $4 for value, $30 generic and $55 for brand drugs. Specialty drugs for a 30-day supply are $15 for generic and $30 for brand drugs.

Kaiser

Kaiser Permanente is the HMO service offered by the city. Employees enrolled in the Kaiser plan, must use Kaiser or Portland Clinic facilities and doctors. There is no annual deduction and employees pay $20 co-pay per visit to primary care, specialty care, and urgent care. Maximum out-of-pocket (Kaiser will pay all costs after you reach this amount) is $750 for individual and $1,500 for family. This plan has an alternative care package which includes chiropractic, naturopath and acupuncture benefits with $15 co-pay and annual maximum benefit to $1,500.

Prescription benefits are available at Kaiser Pharmacies; $15 co-pay for a 30-day supply for generic and preferred brand drugs. Mail order prescriptions are available at $30 co-pay for a 90-day supply for generic and preferred brand drugs.

Vision

Vision is included in the medical plans.

There is no deductible for covered services or supplies and the benefits are paid up to a maximum of $300 every calendar year for all covered services under MODA Health and Kaiser for all groups (BPA, Management and SEIU employees). Payment is based on the contracted fee for in network providers and billed charges for out of network providers.

Dental Insurance

Dental starts the first of the month following your hire date, unless you started on the 1st day of the month, then coverage will begin the day hire. The city offers the following options for insurance coverage:

Delta Dental

The Delta Dental plan, also known as Oregon Dental Service (ODS), is incentive based. Coverage starts at 70% the first year and increases, with annual preventative check-ups, by 10% each year. Employees must use a member dentist to receive full coverage. There is no deductible. The premium is fully paid by the city. Differences in coverage are based upon the employee group as follows:

  • Management employees: Annual maximum for covered services per eligible individual enrolled is $3,000. Orthodontic benefit is paid at 100% up to a lifetime maximum of $1,500 per eligible employee and their covered dependents.
  • SEIU employees: Annual maximum for other covered services per eligible individual enrolled is $2,500. Orthodontic benefit is paid at 100% up to a lifetime maximum of $1,500 for dependent children if treatment begins prior to their 17th birthday.
  • Beaverton Police Association (BPA) employees: Annual maximum for covered services per eligible individual enrolled is $2,500. Orthodontic benefit is paid at 100% up to a lifetime maximum of $5,000 for dependent children if treatment begins prior to their 17th birthday.

Kaiser

The Kaiser plan is an HMO plan, you must visit a Kaiser dentist or facility. There is no deductible and the plan has an unlimited annual maximum. Services are payed at different percentages:

  • Preventive, diagnostic, routine fillings and tooth extractions: 100%
  • Periodontics and Root Canals: 80%
  • Major (implants included): 50% up to a maximum of $2,500
  • Orthodontia: 50% for Adult & Child up to a lifetime maximum of $2,000

Life Insurance

Life insurance is provided through Standard Insurance. The premium for employee basic coverage is fully paid by the city. Employees have the option to purchase supplemental insurance that offers higher limits and dependent coverage. All groups; Management, SEIU and BPA employees has $50,000 in coverage.

Accidental Death and Dismemberment Insurance (AD&D)

Coverage is provided through AIG Life Insurance Company. Coverage differs by employee group. The basic premium is fully paid by the city. Employees have the option to purchase supplemental insurance that offers higher limits and dependent coverage.

  • Management Employees: Basic benefit amount is $151,000 for executive employees; $101,000 for non-executive exempt management employees and $81,000 for non-exempt management employees working 40 hours per week.
  • SEIU Employees: Basic benefit amount is $81,000 for employees working 40 hours per week.
  • BPA Employees: Basic benefit amount is $81,000 for employees working 40 hours per week.
