• Bring application form DS-11 and documentation such as your state certified birth certificate (must list parents' names) or naturalization certificate
• Sign the application in person at an authorized passport acceptance facility
• The facility mails your passport application and submitted documents to U.S. Passport agency for processing
• You will receive your new passport and submitted documentation in the mail
