First time applicants need to apply in person. All children, including infants, are required to appear. Students ages 16 to 17 should be accompanied by a parent or legal guardian. Please bring the following items to process your passport application:

  • Two identical passport photos (2 inches by 2 inches). These photos can be obtained through a travel agent, a photography studio or a photo processing center. Please be sure to ask for passport photos as they are processed on special paper with a special background.
  • Proof of citizenship. A certified copy of your birth certificate issued from the State Registrar’s Office of the State in which you were born, a previously issued U.S. passport, or a certificate of naturalization. A hospital certificate is not acceptable, nor is a notification of birth record.
  • Applicants who possess a California abstract birth certificate should be aware that this document is not acceptable for passport purposes. Applicants who possess a Texas abstract birth certificate issued in border counties should be aware that this document may not be acceptable for passport purposes. If you have any questions about the documents you have, please call our office or bring the document to our office so that we may see what you have and advise you on how to proceed. For applicants born in Oregon who need a copy of their birth certificate, this information can be obtained from the State Vital Records Office.
  • Current picture identification. A driver’s license or an ID card issued through the State.

Be advised that the Department of State requires all payment in the form of check or money order, so please have this form of payment with you when you apply.
